Health Insurance in Spain with pre existing conditions

As you are seniors in the 65+ age group, it is likely that you have had a little more time to acquire a pre existing condition.  You may be in fear of not being able to obtain insurance to meet the residency requirements. This doesn’t mean you won’t be able to obtain health insurance. 

Most Spanish health insurance companies have a medical review process during the application period.  You will likely answer a series of questions about your medical history and pre-existing conditions.  From this review, there are typically 3 different outcomes:

  1. You are approved!
  2. You are approved, excluding a pre existing condition.
  3. You are declined

In rare instances, you may pay a higher premium for coverage, but often this is not possible. 

Please be sure you are always honest and forthcoming with your medical questionnaire.  We have known of people who were not, so they could obtain coverage.  An incident occurred and they went to the hospital and the pre existing condition was discovered.  They were immediately terminated from their health insurance plan and needed to pay for all of their care out of pocket.  Don’t take the risk.

 

Healthcare Spain Expats & Retirees

Okay, now I am going to also let you in on a little secret.  Some Autonomous communities in Spain allow foreigners to register with the public health system (Sistema Nacional de Salud or SNS). 

This program is called “convenio especial sanitaria“.  For those of you who may have pre existing conditions that were excluded from private health insurance, this may be a good option for you to move towards.  

Keep in mind most private health insurance in Spain companies provide you with a 1-year renewable contract.  They do require a minimum of 30 – 60 days’ notice for cancellation, at your renewal date. Most will not allow you to cancel mid-contract. 

It does take time to apply and be approved for the convenio especial, so plan on having your Spanish private health insurance for 2 years.  This will ensure you meet your residency renewal requirements after your first year.  After which, you may apply for the Convenio Especial and be covered by both for the remainder of your 2nd year. 

General Convenio Especial Requirements
  • Evidence of effective residence in Spain for a continuous period of at least one year immediately prior to the date of application for the special agreement.
  • Proof you are registered in a municipality belonging to the territorial scope of the Autonomous Community.
  • Not having access to a public health protection system for any other reason, either by application of national regulations, community regulations on Social Security, or bilateral agreements that have been signed by Spain with other countries in this area.
Costs to buy into the (Sistema Nacional de Salud) Spanish public health system

The cost varies depending on your age and payments are monthly.  It is usually for those under 65 or for 65+. Each Autonomous community may choose to participate and the price may vary.  

  • If the subscriber is under 65 years of age: a monthly fee of 60 euros.
  • If the subscriber is 65 or over: a monthly fee of 157  to 175 euros.
